Patient Information


The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

New Patients: Please have your insurance information ready, so we can check you benefits prior to your visit. If the patient fails to provide their insurance information before or at the time of appointment, he/she will have to pay for the treatment done and the patient will be reimbursed from the insurance. Please arrive least 15 minutes early to have time to fill out the personal information sheet.

Should it be necessary for you to reschedule an appointment, we require 24hours notice on weekday, and at least 3 days notice on Sunday appointments. This allows us to use your reserved time for another patient. Otherwise, a cancellation/ no show fee of $25.00 for short appointments, and $50.00 for long appointments will be charged. This is due upon your next appointment. We have a very busy practice. As much as we would like to accomodate more patients, some have to wait for a few weeks before they can be booked. This cancellation policy will help us make use of our time more efficiently.

Emergencies: We accept emergencies and walk-ins, but please call ahead to avoid waiting time.

Payment Options

For your convenience, we do accept major credit cards(Visa, Mastercards, Amexco) debit cards and cash.  Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
